SUBSTANCE: stages of total peritonectomy of the right and left iliac regions are performed from the ligated round ligament of the uterus along the lateral edge of the psoas major muscle to the area of intersection of the ureter in the right iliac region with the external iliac artery, in the left iliac region to the intersection of the ureter with the common iliac artery, then down along the separated from the peritoneum of the ureter to the ligated round ligament of the uterus. Then, a strand of the lower pole of the omentum is transposed into the projection of the removed pelvic lymph nodes with tamponing of the obturator foramina with the omentum. Strands of the omentum are fixed on both sides with ligatures to the skeletonized psoas major muscle, to the mesentery of the sigmoid colon and to the peritoneum of the recto-vaginal pouch, the vaginal stump is sutured to form an opening 2 cm long, followed by partial supra-stump peritonization of the small pelvis, for which the peritoneum of the bladder is sutured to the anterior wall vagina, and then the peritoneum of the recto-vaginal pouch with the posterior wall of the vagina.

EFFECT: prevention of the formation of lymphatic cysts in patients undergoing extensive surgical interventions with pelvic lymphadenectomy.
