FIELD: medicine; neurosurgery.
SUBSTANCE: for prevention of liquorrhea and liquorocele in surgical treatment of patients with type I Chiari malformation, median suboccipital decompression craniotomy is performed. Then, the arachnoid membrane is inspected in the area of surgical intervention, and if its defect size is less than or equal to 5 mm, the subarachnoid space is sealed. Z-shaped suture is applied on the area of the detected defect, arranging in-and-out seam parallel stitches at 1 mm from the edges of said defect on each side. Further, a section of fatty tissue is taken in the area of the surgical wound, which is twice the area of the defect of the arachnoid membrane, center of this section is fixed to area of superimposed suture with the help of suture of this suture. Dura mater defect is closed.
EFFECT: method enables preventing the development of liquorrhea and liquorocele and associated complications in the surgical management of the patients with type I Chiari malformation.
