FIELD: medicine.

SUBSTANCE: group of inventions relates to a method for sanitation video-assisted retroperitoneoscopy in treating necrotizing pancreatitis and a device for separation and fragmentation of sequestrates. Method involves ultrasound-guided puncture with a needle with a mandrin located therein, introduction of the metal guide after penetration of the needle into the cavity and removal of the mandrel with subsequent introduction of the dilator and the catheter. Conductor serves as a guide of the device for separation of sequesters from the walls of the purulent cavity and their fragmentation. After the distal end of the device reaches the purulent cavity during its introduction, the conductor is removed and an optical straight tube is delivered through the hollow axis with the screw element. After visualization of sequestrates, the device is brought to the level of holes in the sleeve and by means of adjusting handle with internal thread and torsion spring, lever clamping planks are displaced outwards. Device is rotated and reciprocated around the optical line tube to separate the sequestra from the cavity walls. Thrust handle is brought together with the disk stop, compressing the first spring, to displace the cylindrical thrust-movable tube together with the disk towards the distal end of the device, which leads to rotation of thrust bushing with cylindrical blade knives and fragmentation of sequestrates.

EFFECT: use of a group of inventions enables performing sanitation video-assisted retroperitoneoscopy using one access point with possibility of visual control and adequate sanitation of pathological center.
