FIELD: medicine.

SUBSTANCE: invention refers to medicine, particularly to otorhinolaryngology. At the first stage, under intubation anaesthesia, the patient undergoes infiltration anaesthesia in the area of the auricle antitragus, and a skin-cartilage graft is taken. At the second stage, infiltration anaesthesia is performed using Articaine in the area of the quadrangular cartilage. Then, a vertical incision is made in the caudal region of the quadrangular cartilage from the tip of the nose to the bottom of the nasal cavity, and the residual mucoperichondrium and mucoperiost of the quadrangular cartilage are separated with a sharp raspatory. At the third stage, the transplanted graft is fixed in the bed using interrupted sutures; the graft is sutured so that the cartilaginous portion is matched with the remainder of the quadrangular cartilage. Skin portion is placed between the cartilage and mucoperichondrium and then fixed with sutures on both sides. After the graft is fixed, gauze tampons lubricated with antibacterial ointment are placed in the nasal cavity. On the following days, antibacterial and regenerating ointments are applied on the operated area for 10–14 days.

EFFECT: method enables providing the graft engraftment and a good cosmetic result in the long term in the reconstruction of the nasal columella in congenital and acquired defects of the external nose in children.
