SUBSTANCE: preoperative preparation of the patient is carried out, then a cut along the transitional fold before the defect to the entire height of the columella to the nasal cavity bottom is made on both sides under the intubation anesthesia and infiltration anesthesia in the columella area and the nasal cavity bottom near the vestibule. Tissue is incised along the demarcation line. A bed for a transplant is prepared. Then a transplant is formed, for this, the inferior nasal concha mucosa is infiltrated with an anesthetic solution and the mucosa is excised throughout the inferior nasal concha with the capture of the osseous part of the concha in its middle section, 0.3 cm departing from the anterior end of the inferior nasal concha. The transplant is immediately transferred to the prepared bed and sutured with resorbative materials proximal to the columella, the distal end of the nasal concha transplant is placed between the mucoperiost sheets on the perpendicular plate of the sieve bone and fixed. In the same way, reconstruction is carried out from the opposite side.

EFFECT: method allows to eliminate pathological changes in tissues due to the closure of the nasal septum defect, reducing the severity of inflammation, inhibiting the progressive sclerotic process, preventing formation of repeated perforations, and restoring the laminar flow of inhaled air and normalizing patient's immunological parameters.
