FIELD: medicine; plastic and maxillofacial surgery.
SUBSTANCE: subperichondrial and subperiosteal detachment of soft tissues is performed from the lower and the upper lateral cartilages to the root of the nose. Next, a parallelepiped-shaped graft with smooth edges is formed from the costal cartilage and placed on the formed domes of the tip of the nose. A crushed graft is placed on top, which is formed from a quadrangular plate of nasal septal cartilage using a Cottle endotome. Next, the mucocutaneous flap is placed in place and sutured. Air-conducting intranasal splints are installed and fixed into the nasal cavity with one interrupted suture. A bandage made of narrow and wide strips is applied to the nose. The external thermoplastic nasal splint is fixed.
EFFECT: method allows to recreate the anatomical structures and restore the projection of the terminal part of the nose in patients with postoperative deformities of the osteochondral part of the nose.
