FIELD: medicine; surgery.

SUBSTANCE: when performing a dilated tracheostomy under the control of a fiber-optic bronchoscope, the level of dilated tracheostomy is additionally determined by percutaneous transillumination. Trachea is punctured with a needle with a plastic cannula. Cannula is inserted into the tracheal lumen. Needle is removed. Guide wire is delivered through the cannula. Cannula is removed. Skin is incised 2 mm in horizontal plane with a scalpel. Soft tissues are expanded with a predilator. Stoma is formed with one cone-shaped dilator. Tracheostomy tube is placed along the guidewire and a continuous blanket percutaneous suture is applied there around.

EFFECT: method enables to additionally determine the level of tracheostomy, preventively protecting patients from developing complications in the form of profuse haemorrhage with comorbid coagulopathies and with underlying use of therapeutic dosages of blood thinners, reduced risk of decannulation, improved functional and cosmetic results of treatment due to less traumatization of skin.
