FIELD: medicine.
SUBSTANCE: invention relates to maxillofacial, aesthetic and reconstructive surgery. A separate intraoral approach is performed in the lower fornix of the oral vestibule with preserving the cuff of the unattached mucosa. Then, a limited periosteal detachment is performed only in the projection of the planned osteotomies. That is followed by a monocortical osteotomy of the external surface of the jaw parallel to its occlusal plane, which separates the boundaries of the mobilized bone fragments. That is followed by a bicortical horizontal osteotomy of the chin perpendicular to the bone surface to projection points of the loops of the inferior alveolar nerves. Further, the bicortical osteotomy is connected to the monocortical osteotomy in the region of the chin holes. A bicortical osteotomy is performed in the posterior parts of the lower jaw, the line of which is connected to the line of the monocortical osteotomy. Then by means of chisels primary mobilization of fragments of bone material is performed with possibility of opening diastases in area of sawn external cortical plate up to 2 mm. Then the bone fragment is mobilized. In the lateral region, forces are applied laterally, starting from the level of the external oblique line with gradual advancement backwards. In the chin, forces are applied vertically with a directed fracture of the lingual cortical plate within the body and branches of the lower jaw. Bone diastasis is examined for integrity of lower alveolar nerves. Thereafter, the distal fragment is finally mobilized to achieve its mobility and positioning. Further, osteosynthesis is performed.
EFFECT: method enables to correct the contours of the lower jaw and chin with insufficient manifestation, asymmetry of the contours of the lower one-third of the face, reduces risk of damage to lower alveolar and mental nerves during osteotomy, provides even and smooth contours of lower jaw, minimizing costs for preliminary planning of operation, reduces risk of thinning and fracture of extracted fragment of lower jaw in undesirable place.
