SUBSTANCE: invention relates to the field of medicine, namely, maxillofacial surgery, and is intended for use in the treatment of patients with gnathic forms of dentoalveolar anomalies, which shows a combined treatment with the use of orthognathic surgery techniques. Conduct a cut from the inner surface of the branch of the lower jaw, above or below the entry point of the mandibular nerve, on an oblique line with the transition to the external side of the body of the lower jaw and continuing to the projection of the eighth, seventh or sixth molars of the lower jaw. After that, a vertical cutting is performed on the body of the lower jaw by 1/3…1/2 of the height of the body of the lower jaw. Then a horizontal cutting is performed parallel to the lower edge of the body of the lower jaw with an indent 1 cm from the lower edge, in the direction from the posterior edge of the branch in the region of the angle of the lower jaw to the previous vertical incision in the projection of the eighth, seventh or sixth molars. At the same time, all cuttings are carried out on the entire thickness of the cortical layer without cutting into the spongy layer. Similar manipulations are carried out from the opposite side of the lower jaw. After the intercortical osteotomy of the lower jaw, a large fragment and two small fragments are formed. Then a large fragment using the previously made splint (positioner) is fixed to the upper jaw in the orthognathic ratio. Small fragments are positioned and fixed to the large titanium plates and screws.

EFFECT: method allows to recreate the right angle between the branch and the body of the lower jaw and to achieve the correct aesthetic contour of the lower third of the face.
