FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to surgical dentistry, and can be used in treating Miller class III and IV gum recessions in anterior mandible. After preliminary rinsing of patient’s mouth with 0.12% solution of chlorhexidine gluconate for 30 seconds, contact points are splinted with composite. Then, under infiltration anaesthesia, using a flexible microsurgical blade VIPER, intra-sulcus incision is performed from vestibular and lingual sides in the area of operated teeth of lower jaw, with obligatory inclusion of one tooth on both sides. Further, a tunnel is formed in the mucosa from the vestibular and lingual sides; muscle and collagen fibres are attached apically and laterally to the inner surface of the tunnel; mobility of the flap is checked with a periodontal probe. Two free gingival grafts are sampled, where one corresponds to the length of the vestibular tunnel, and the second one corresponds to the length of the lingual tunnel; the height of autografts is 3–4 mm, thickness is 1–2 mm. Further, the transplants are de-epithelized on the operating table for 15 s, after which the de-epithelialized grafts are immersed in the vestibular and lingual tunnels, respectively, and fixed with interrupted sutures. Tunnel flaps are displaced together with the immersed autografts coronal and fixed with double crossing sutures, followed by oral baths with 0.12% chlorhexidine bigluconate twice day for 4 weeks. Sutures are removed in 14–21 days, in 4 weeks the patient is recommended to start brushing with a soft toothbrush using the modified Bass technique, followed by control examinations.
EFFECT: method enables increasing the effectiveness of the surgical management of the Miller class III and IV recession of the lower jaw, increasing the volume of the interdental papilla and the attached gingival area by creating simultaneously two tunnels from the vestibular and lingual sides and introducing two de-epithelial flaps with subsequent fixation.
