FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to dentistry, and can be used in surgical elimination of multiple gingival recessions. Cone-beam computed tomography is performed comparing the areas of teeth with gingival recessions and without recessions. Bone tissue resorption value is determined, the gingival recession area is detected, and the tooth with maximum recession is determined. Performing local infiltration anaesthesia and performing incisions in the area of gum recession, wherein with gum recession up to 3 mm, incisions are made along the gingival edge to the papillae, without affecting them, and in case of recession from 4 mm, oblique paramarginal incisions directed converging to the tooth with maximum recession are made from the median point of the apex of the gingival edge of the adjacent tooth to the line of the cement-enamel junction in the area of the tooth with maximum recession. Further, the full-thickness mucoperiosteal flap is folded back and a submucosal tunnel is formed to perform tunnel splitting of tissues without dissecting the gingival papillae. Then, tooth root surface is machined and polished. Then a connective tissue autograft is taken from the donor area of the hard palate, followed by placing the haemostatic sponge on the donor area of the hard palate and closing the wound with cross sutures. Produced autograft is de-epithelized and shaped to the size of the created submucosal tunnel. After achieving unobstructed mobility of the flap by dissecting all muscular and mucous cords, the prepared autograft is laid in the created tunnel, stretching inside the split flap, and fixed with interrupted sutures to the periosteum. Split tunnel is fixed with double blanket sutures, additionally piercing the autograft, the flap is coronally displaced and fixed with double blanket sutures, an interrupted suture is applied in the area of the detached gingival papillae with suturing from the palatal surface. Further, double blanket sutures are applied in the area of each tooth with simultaneous piercing of the transplant, and a periodontal bandage is applied.
EFFECT: method enables providing the maximum aesthetic and functional result in eliminating multiple gingival recessions when the depth of recessions in the area of each tooth is significantly different.
