FIELD: medicine.
SUBSTANCE: invention relates to vascular surgery. 5.0–6.0 cm long incision is made from the angle of the lower jaw to the level of the thyroid cartilage. Internal jugular vein with a facial vein is separated, a fascial sheath of the sternocleidomastoid muscle is opened and laterally removed; the internal jugular vein is abducted. After separating the internal jugular vein, the facial vein is separated, the facial vein is ligated and retracted outwards, wherein the internal carotid artery is separated in the distal direction from the atherosclerotic plaque, and the common carotid artery is separated in the proximal direction from bifurcation at 3.0–4.0 cm, the external carotid artery, above bifurcation by 1.0–2.0 cm, preserving the structure of the carotid glomus. Then, clamps are applied on the highlighted sections of the arteries, the common carotid artery is transected below bifurcation by 1.5–2.0 cm and a carotid eversion endarterectomy is performed by exfoliation of an atherosclerotic plaque from the internal and external carotid arteries, then the atherosclerotic plaque is detached from the common carotid artery to its residual height of 3.0–4.0 cm. Atherosclerotic plaque is dissected away and anastomosis is applied.
EFFECT: method enables to bypass the area of the carotid glomus and eliminate traumatisation of cranial nerves within the surgical wound, due to which it is possible to avoid intra- and postoperative hemodynamic complications in the form of arterial hypertension, reduce the length of the surgical intervention and eliminate neurological complications of cranial nerves.
