FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to cardiovascular surgery. An eversion carotid endarterectomy (EAE) is performed to remove the atherosclerotic plaque all along and with subsequent anastomosis. At that, after plaque removal, the adequacy of endarterectomy is evaluated, in case of floatation areas of intima on border of endarterectomy, or resedual stenosis, or prolonged atherosclerotic plaque, or artery dissection, an internal carotid artery is dissected longitudinally in the middle of the medial semicircle in the distal direction to an edge of endarterectomy. An arteriotomy edge on the ICA is then projected onto the ECA wall. An ECA arteriotomy is performed along a side wall in a distal direction to an ICA arteriotomy with subsequent ICA replantation. Anastomosis is formed by continuous blanket suture with needle prick-in and needle stick-out from inside to outside.
EFFECT: method enables providing the most anatomical and hemodynamically physiologic carotid artery bifurcation by using the ECA dissection in the distal direction with the formation of the heel and the initial anterior and posterior lip of the anastomosis at a certain distance, and also enables to perform adequate EAE due to prolonged arteriotomy and audit of the EAE area and avoid carotid artery prosthesis using synthetic materials, which reduces the risk of thrombosis of the anastomosis area and reduces the risk of postoperative complications.
