SUBSTANCE: cut in the internal carotid artery mouth is carried out. An eversion endarterectomy from the internal carotid artery is performed followed by reimplantation into the common carotid artery. After an eversion endarterectomy, semi-closed endarterectomy of the common carotid artery is performed. For this, additional transverse arteriotomy of the common carotid artery is performed, which is carried out along the anterior-lateral wall of the common carotid artery at a distance of at least 5 mm from the proximal end of the atherosclerotic plaque. The length of the additional arteriotomy is not less than 1/3 of the common carotid artery circumference. Then, the transcutaneous arteriotomy is sutured before internal carotid artery reimplantation.

EFFECT: method reduces the risk of microembolia in the internal carotid artery pool during surgical treatment, reduces the risk of postoperative complications such as acute cerebrovascular accident, impaired cognitive function, and reduces the time of intraoperative cerebral ischemia.
