FIELD: medicine.
SUBSTANCE: invention relates to surgical ophthalmology and can be used for inspection of non-penetrating deep sclerectomy in its blockade by root of iris with one-stage phacoemulsification of cataract. That is ensured by making a primary incision of the cornea with width of 2.0 mm at eleven o’clock, and two paracenteses with width of 1.0 mm at nine and three o’clock. 0.1 ml of 1 % phenylephrine is introduced into the anterior chamber. Then front chamber is filled with adhesive viscoelastic. Capsulorhexis with diameter of 5.0 mm is performed. Hydrodissection and hydrodelineation of lens are performed. Cataract phacoemulsification is performed. Cortical masses are aspirated. Capsule sac, posterior and anterior eye chambers are filled with cohesive viscoelastic. Intracapsular ring and an intraocular lens are implanted into a capsular sac. Main incision is hydrated. Viscoelastic is washed out with an aspiration-irrigation system. 0.2 ml of 0.01 % carbachol is introduced into the anterior chamber. Through the paracentesis, 0.1 ml of cohesive viscoelastic is introduced into the anterior chamber in the projection of the NPDS area above the root of the iris, pressing the iris backwards. Then under control of gonioscopy cannula with included irrigation flow is pulled back to the iris downwards and to the pupil, freeing up the operation area. Trabeculotomy is performed through the paracentesis in the area of the NPDS. Further, through the anterior chamber, a spatula is inserted under the surface flap and the adhesions are separated by a blunt method. Operation is completed by hydration of corneal incisions.
EFFECT: invention makes it possible to improve outflow of intraocular fluid, to expand angle of anterior chamber, to reduce risk of repeated blockade of area of antiglaucomatous operations, to improve visual functions, reduce traumatization of eyeball tissues, reduce length of medical and social rehabilitation.
