FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to abdominal surgery, and can be used to assess the functional state of the liver in obstructive jaundice with underlying ultrasound-guided percutaneous transhepatic cholangiostomy. When forming a primary puncture approach to the bile duct, a portion of bile is additionally sampled, a bile drop is applied on a slide, dried at room temperature and minimal air movement, and a crystallographic examination is carried out using a microscope. Bile is sampled over time from a drainage cholangiostomy catheter. Drop of 0.01 ml of bile is dried for 5 hours, and the data of the crystallographic study are assessed by assigning points according to the criteria for determining the homogeneity and ordering of the crystallization zone, as well as the angle of inclination of the liquid crystal lines in accordance with the description table. If total score is 3 to 5, preservation and/or complete recovery of liver function is stated. If total score is from 6 to 9, hepatic dysfunction is stated. If total score is from 10 to 12, the conclusion is made on progression of liver dysfunction and, accordingly, on severe course of hepatic insufficiency.
EFFECT: method provides the possibility of providing a dynamic assessment of the functional state of the liver in patients with obstructive jaundice syndrome while eliminating the disadvantages of laboratory diagnostics: long analysis time, the need to use expensive specialized equipment and reagents; and reducing its labor intensity, which will make it possible to assess the functional state of the liver in each patient with obstructive jaundice syndrome, selecting the most effective therapeutic approach individually for each patient and monitoring the effectiveness of the surgical intervention by crystallographic analysis of the dried bile drop.
