SUBSTANCE: invention relates to plastic surgery, otorhinolaryngology. With the help of a round knife or a monopolar electrode, an incision is made from the shoulder of the inferior nasal concha medially towards the septum to the middle of the bottom of the nasal cavity. Then the incision is made along the nasal septum and extended posteriorly by 1.5–2 cm. Formed triangular flap is dissected by means of a raspatory or a round knife. Flap is lifted and folded back to visualize the bottom and lateral wall of the inferior nasal passage. Then, using a piezosurgical apparatus and a sharp chisel, a lower longitudinal incision of the bone of the lateral wall is made along the bottom of the nasal cavity. At 5 mm above the lower incision and parallel to it, an upper longitudinal bone incision is made. Then two longitudinal incisions are combined with an anterior transverse incision. Bone graft is sampled. Formed bone window and the defect of the maxillary sinus mucosa are closed in the lower nasal passage by placing the triangular flap back. Flap is straightened and sutured with two interrupted sutures in the shoulder area of the inferior nasal concha. Further, the bone graft is prepared. Bone graft is cut with scissors, turned with a bur or a piezosurgical apparatus to the desired shape. Then the bone graft is perforated with a bur with a thin cutter or an awl of a piezosurgical apparatus.

EFFECT: method enables obtaining and preparing a bone graft from the lateral wall of the inferior nasal passage and using it in rhinoplasty, reducing the risk of postoperative complications.
