FIELD: medical science.

SUBSTANCE: group of inventions refers to medicine, namely to plastic surgery. Variants of rhinoplasty technique with use of sleeve-type autograft from auricular perichondrol. In the first version of the rhinoplasty technique, an autograft is first prepared. That is ensured by subcutaneous hydrodissection, incision and skin detachment of a posterior surface of a donor auricle. Perichondrium is separated from the auricle cartilage and horizontally dissected away along the auricle base. Skin wound is closed. Rectangular portion is cut out from the dissected perichondrium, the length of which corresponds to the length of the nasal dorsum, and the width is not less than twice the desired width of the nasal dorsum. Perichondrium is folded and sutured laterally to produce an autograft in the form of an empty sleeve at the end of which free guide sutures are left. At the final stage of the rhinoplasty, after nasal skin and soft tissues are separated from cartilages and bones of the nasal skeleton, the nasal dorsum is enlarged and camouflaged with an autograft. Nasal elevator is inserted under the skin and soft tissues of the nose. Needles fixed on opposite ends of guide sutures are used to deliver guide sutures on skin surface within nasal root on the right and left by skin puncture from inside to outside. Guide sutures are pulled, and the autograft is introduced under the visual control under the skin and soft tissues of the nasal dorsum. Autograft is sutured. Incision is closed, the nasal cavity is packed, and a splint is applied. According to the second version of the rhinoplasty method, perichondrium is incised and dissected away in a similar manner. Auricular cartilage is sampled and ground. Perichondrium is folded and sutured on the sides to produce an autograft in the form of an empty sleeve. One end of the sleeve is sutured together. Sleeve is filled with the ground cartilage uniformly introduced into the sleeve from the unsutured end. Then the second end of the sleeve is sutured together; an autograft in the form of a filled sleeve is prepared with free guide sutures left at the end. At the final stage of the rhinoplasty, after nasal skin and soft tissues are separated from cartilages and bones of the nasal skeleton, the nasal dorsum is enlarged and camouflaged with an autograft.

EFFECT: method allows to reduce injuries and the likelihood of visible cicatrisation in a patient during rhinoplasty, to eliminate the risk of developing alopecia in the donor zone, as well as the risk of damage to the trunks of sensory nerves of the auricle, to form an autograft for the camouflage of irregularities and defects of the back and tip of the nose, depending on the size of the defects.
