FIELD: medicine.
SUBSTANCE: invention relates to hepatobiliary surgery. An antegrade transfistula approach is formed on a T-shaped drainage of the common bile duct for 21–40 days. Under X-ray control, an angiographic conductor is introduced through the lumen of the T-shaped drainage, and the T-shaped drainage on the guidewire is removed. A choledochoscope is inserted into the bile ducts parallel to the conductor and concrements are detected. Small concrements are removed with Dormia basket, large concrements are removed in fragments after mechanical, electrohydraulic or laser lithotripsy. A direct drainage is installed, directed towards an ampullary portion of a common bile duct, a control cholangioscopy and a cholangiography are performed, in 1–2 days the drainage is removed.
EFFECT: method increases the clinical effectiveness in the patients with cholangiolithiasis in the patients with the T-shaped drainage of the common bile duct, provides the possibility of complete removal of all calculi, including large and multiple ones, regardless of their composition and density, which provides reduction of postoperative complications and lethality, reduction of operative injuries, preservation of autonomy of biliary tract due to antegrade transfistula approach formed on T-shaped drainage of common bile duct under control of cholangioscopy.
