FIELD: medicine.
SUBSTANCE: rod-type transosseous fixation apparatus for extrafocal osteosynthesis for reduction and stabilization of pelvic bone fragments in fractures passing through the cotyloid cavity, comprises an anterior component, a posterior component, two longitudinal connecting threaded rods and a femoral component. Anterior component is made of a first longitudinal threaded rod for the right half of the pelvis, a second longitudinal threaded rod and two or more reposition-fixing assemblies, made in the form of first slides with two mutually perpendicular holes at a distance from each other with possibility of fixation on said first and second longitudinal threaded rods with nuts in one of the slider holes and fixation with nuts of the transverse threaded rod in the second hole of the slider and in the form of second slides with two mutually perpendicular holes at a distance from each other with possibility of fixation by nuts in the first hole of the slider on the transverse threaded rod and fixation with nuts in the second hole of the slider of the transosseous element in the form of a Schanz screw with a thread on the fastening end. Transosseous elements can be arranged in different planes. Two said, first and second, longitudinal threaded rods can be attached to each other at an angle by a connecting assembly open towards the pelvis, comprising a transverse rod and two slides with two mutually perpendicular holes at a distance from each other, one of which in the first hole is fixed with nuts on said first longitudinal threaded rod, and the second slider is fixed in the first hole with nuts on said second longitudinal threaded rod, in the second holes of these both sliders a transverse threaded rod is fixed. Posterior component is made of a longitudinal threaded rod with two reposition-fixing units in the form of the first sliders with two mutually perpendicular holes at a distance from each other with possibility of fixation on longitudinal threaded rod with nuts in one of holes of slider and fixation with nuts of transverse threaded rod in second hole of slider, the second slides have two mutually perpendicular holes at a distance from each other with the possibility of fixation on the transverse threaded rod with nuts in one of the slider holes and fixation with the Schanz screw nuts in the second hole of the slider. Schanz screws of both reposition-fixing assemblies are located in one plane, front and rear components can be connected to each other on the sides on the right and on the left by two longitudinal connecting threaded rods. Anterior component on the sides on the right and on the left can be connected to two longitudinal connecting threaded rods by means of slides with two mutually perpendicular holes at a distance from each other. In the first hole, a longitudinal threaded rod of the front component is fixed on the free side end, in the second hole of the slider a longitudinal connecting threaded rod is fixed, posterior component on the sides on the right and on the left has the possibility of being attached to two longitudinal connecting threaded rods by means of connecting assemblies, each of which includes a transverse rod and two slides with two mutually perpendicular holes at a certain distance relative to each other, one of which in the first hole is fixed with nuts on the longitudinal threaded rod of the fixation unit of the posterior semi-ring of the pelvis, and the second slides are fixed in the first hole with nuts on the longitudinal connecting threaded rods, in the second holes of these both introduced slides a transverse threaded rod is fixed on the right and on the left. Femoral component is made of a longitudinal threaded rod with two reposition-fixing assemblies in the form of first slides with two mutually perpendicular holes at a distance from each other with possibility of fixation on longitudinal threaded rod with nuts in one of holes of slider and fixation with nuts of transverse threaded rod in second hole of slider, second slides with two mutually perpendicular holes at a distance from each other with possibility of fixation on transverse threaded rod with nuts in one of the slider holes and fixing the transosseous element made in the form of a Schanz screw in the second hole of the slider with nuts. Transosseous elements of both reposition-fixing units can be located in one vertical plane along the outer surface of the thigh. Femoral component can be connected to the longitudinal connecting rod by means of a slider with two mutually perpendicular holes at a distance from each other, in the first hole it is fixed with nuts on the longitudinal threaded rod of the femoral component, and in the second hole it is fixed with nuts on the longitudinal threaded connecting rod.
EFFECT: invention provides higher efficiency of reduction and stabilization of fragments in extrafocal transosseous osteosynthesis, reduced time for operation, possibility of delayed reduction, reduced time of postoperative recovery and improved patient’s quality of life.
