FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to X-ray endovascular diagnosis and treatment, and can be used for transcatheter aortic valve implantation (TAVI) using a new arterial vascular approach. Axillary artery is separated, its proximal and distal segments are taken on holders, subscapular artery is separated in the area of origin from axillary artery. A longitudinal arteriotomy of the subscapular artery is carried out, introducer 10 Fr is brought through the arteriotomy opening of LA to the lumen of the axillary artery, diagnostic catheter AL2 is inserted along the conductor J into the artery and then into the ascending aorta; the conductor J is replaced with a straight conductor; the conductor is delivered from the aorta into the left ventricular cavity. A diagnostic catheter AL2 is inserted along the conductor into the left ventricle; the inserted straight guide is replaced with 260 cm long J guide; then, the AL2 catheter is replaced with a pig-tail catheter. Super-rigid conductor Confida 0.035 is inserted into the left ventricle, the pig-tail catheter is removed, along the installed conductor Confida 0.035 in the LA and further into the projection of the aortic valve, the aortic valve is introduced by means of delivery system 18 Fr, an aortic valve is implanted; a delivery system is removed from the subscapular artery; the subscapular artery is ligated at the point of its origin from the axillary artery.
EFFECT: method enables transcatheter implantation of the aortic valve using the subscapular artery approach when the classical transfemoral approach is not possible.
