FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to reconstructive plastic surgery. Mastectomy is combined with separating the vascular graft of the deep lower epigastric vessels with the vascular graft taken as distally as possible. Then, marking is carried out to isolate the perforated lumbar flap, marking the median line passing along the spinal column, retreating from the median line by 7-9 cm to the right and left sides, with pre-marking under the control of ultrasound or CT with contrasting, then in a standing position looking for Crista iliaca landmarks and marking their location area with a marker. Thereafter, a pinch test is performed within the perforating vessel with marking a flap intake area, and an elliptical lumbar flap is separated by incising with 15- or 21-diameter scalpel. Then, dissection is performed from the medial to lateral edge along the thoracolumbar fascia, reaching the perforator, the flap is dissected by means of binocular lenses, after which the perforator is cut off, and the incision of the thoracolumbar fascia is sutured with polypropylene suture 2-0, then the donor site is sealed by means of reducing sutures. Anastomosis is created between the perforating vein and the vascular graft with using 10-0 polypropylene suture; that is followed by creating the anastomosis between the internal mammary vessels and the vascular pedicle of the graft, and the operation is completed by modelling the graft and layered wound closure.

EFFECT: method allows to close the breast defect with the help of pathological reconstruction in women who are contraindicated in the use of a DIEP flap, provides adequate filling volume and shape of mammary glands due to similarity in composition with lumbar fat, provides stable aesthetic effect, as well as improves quality of life of patients.
