SUBSTANCE: invention relates to the field of medicine, to plastic, maxillofacial and ENT surgery and can be used in the treatment of patients with congenital and acquired deformities of the osteochondral portion of the nose to recreate the anatomy and aesthetics of the terminal portion of the nose. A rectangular graft with even edges 2 mm wide, 10–15 mm long and 1 mm thick is formed from the pre-collected septal autocartilage. Straight needle holder is used to press the graft edges at 2–3 mm from the edge. Then the formed graft is placed on the remodelled nose tip and sutured with interrupted sutures. Further, the mucocutaneous flap is placed in place, the resulting aesthetic result is visually assessed and sutured, air-conducting intranasal splints are installed in the nasal cavity and fixed with one interrupted suture, which are removed on an outpatient basis on the third day. A bandage of strips is applied on the nose, an external thermoplastic nasal splint is fixed, which is removed on an outpatient basis on 14th day.

EFFECT: method enables creating an optimal projection of a nasal tip in anatomically thin skin, can be used in patients with congenital and acquired deformities of the osteochondral portion of the nose.
