FIELD: medical science.
SUBSTANCE: invention refers to medicine, namely to ophthalmology, and aims at treating open-angle glaucoma. Superficial scleral flap is formed; the biomaterial Alloplant is laid on the scleral bed bottom; an intraocular fluid outflow is created from the anterior portion to the posterior portion into the suprachoroidal space. First, from the Alloplant spongy biomaterial a rectangular plate of 5×2 mm is formed, which is dissected in the longitudinal direction by half the length, thereby obtaining two free ends, and after the conjunctiva is incised, the superficial scleral flap is formed with an extension of up to 1 mm into the transparent layers of the cornea. Further, under the superficial sclera flap, a deep scleral flap is cut out and after the annular ligament appears, the corneal stroma is bluntly separated, exposing the Helmet's canal and a portion of the trabeculo-Descemet's membrane, wherein the corneal part is dissected, followed by incising the corners of the deep scleral flap, and after removing the outer layers of the trabecula, the deep scleral flap together with the corneal tissue is cut off. Longitudinal incision is made in a scleral bed in a projection of a free edge of the superficial scleral flap up to the choroid, and a spatula is used to separate the choroid to form a suprachoroidal pocket. Formed biomaterial is laid on the bottom of the scleral bed with covering a trabeculo-Descemet's membrane on one side; on the other hand, the first free end of the biomaterial is put into the pre-formed suprachoroidal pocket, and the second end is left on the sclera surface in the sub-Tenon's space. Then superficial scleral flap is placed on top of biomaterial and conjunctival suture is made.
EFFECT: method enables fast rehabilitation of the patients, reducing postoperative complications, ensuring a stable hypotensive effect in the remote postoperative period by combining the two outflow tracts with no risk of postoperative choroidal detachment.
