FIELD: medical science.
SUBSTANCE: invention refers to medicine, namely to traumatology and orthopaedics, and can be used for surgical management of anterior shoulder instability. That is followed by transsubscapular intra-articular tenodesis of the tendon of the long head of the biceps muscle of arm with using knotless anchor fixators and refixation of the glenoid labium of the glenoid cavity of the shoulder blade to the tendon. Two fixation devices are used for tenodesis. Groove 2 mm deep, wide equal to a tendon thickness, and long equal to a distance of the retainers is formed on an anterior edge of the glenoid cavity of the scapula. First retainer is positioned within an attachment point of the middle humeroscapular ligament to the glenoid cavity of the scapula, while the second retainer is positioned within the attachment area of the anterior portion of the lower humeroscapular ligament to the glenoid cavity of the scapula. First, the tendon end is fixed with a first retainer to a front edge of the glenoid cavity of the scapula. Tendon is placed in the formed groove, and a distal portion of the tendon is fixed with the second retainer to a front edge of the glenoid cavity of the scapula. Middle humeroscapular ligament is underrun and fixed to the tendon with the glenoid labium of the glenoid cavity of the scapula with the sutures of the first fixation device. Anterior portion of the inferior humeroscapular ligament is underrun and fixed to the tendon with the glenoid labium of the glenoid cavity of the scapula with the sutures of the second fixation device. Anatomic position of the middle humeroscapular ligament and the anterior portion of the lower humeroscapular ligament is preserved.
EFFECT: method provides reducing risks of graft instability and rejection; preservation of the anatomical location of the middle humeroscapular ligament and the anterior part of the lower humeroscapular ligament; reducing the amount of loss of bone tissue of the glenoid cavity of the shoulder blade due to additional fixation of the middle humeroscapular and anterior portions of the lower humeroscapular ligaments and the glenoid labium of the glenoid cavity of the scapula with the sutures that were used for tenodesis of the tendon of the long head of the biceps muscle of arm and using only two knotless anchor fixators.
