FIELD: medicine. SUBSTANCE: method involves accessing frontal boundary of abducting femoral muscles and deflecting them upwards. Articulation capsule is accessed and longitudinal capsulotomy is carried out. Femur head is dislocated into the wound. Osteotomy is carried out near its base. Head together with cervix are removed from the cotyloid cavity. Cotyloid cavity revision is carried out. Its cavity is finished using mill. The removed femoral head is shaped with spherical mill making cartilage resection to subchondral plate. The cotyloid cavity area is filled with cement and femoral head is placed inside. Full immobility being achieved, neck osteotomy is carried out at subcapital level so that filed bone surface inclination plane is brought in correspondence with normal inclination of entrance plane of the healthy cotyloid cavity. Bed is formed for endoprosthesis cup in the bulk of the preimplanted bone head. Then total endoprosthesis-setting process is started by fixing cup and limb of the endoprosthesis using any known method. EFFECT: enhanced effectiveness of treatment; reliable fixation of cotyloid component.
