FIELD: medicine, operative surgery. SUBSTANCE: in area of an jugular fossa one should bluntly form a foramen into anterior mediastinum with the help of a Fyodorov's clamp. Sternotomy is started from an angle parallel to the body by cutting a circumferential lamella and spongiose layer under a finger control along the whole sternal length up to posterior cortical lamella. The latter is crossed with a guillotine sternotome in area of a handle. At the level of the first intercostal space one should insert the branches of stomatological mouth dilator to separate the edges for 5-10 mm. In retrosternal cellular tissue of sternotomy area a canal is formed with fingers. The cutting of posterior cortical lamella is carried out under a sharp angle to a sternum under a finger control from the side of anterior mediastinum by replacing a mouth dilator into other intercostal spaces, if necessary.
