SUBSTANCE: the present innovation deals with treating transverse foot flatness along with great toe's valgus deformation. It is necessary to dissect excessive osseous-cartilaginous formations in the region of the 1st metatarsal bone caput, dissect the part of a tendon referring to adductor muscle of great toe. One should perform myotransposition of transverse portion of a tendon referring to adductor muscle of great toe towards the 1st metatarsal bone caput, carry out mobilization of sesamoid bones due to external capsulotomy of the 1st metatarsophalangeal joint, remove excessive varus position of the 1st metatarsal bone due to squeezing the foot's anterior department. Through the bottom of the 1st metatarsal bone one should apply Kirschner's needle by stabilizing it against excessive varus position, then comes wedge valgusing osteotomy of proximal metaphysis of the 1st metatarsal bone being distally against the applied Kirschner's needle to remove its residual adduction and derotation. On correcting the 1st metatarsal bone it is necessary to apply the second Kirschner's needle through its distal fragment. The innovation provides the reconstruction of the shape and the function of foot's anterior department, the decrease of the dissected wedge, the decrease of excessive shortage of the 1st metatarsal bone, the exclusion of postsurgical complications.
