SUBSTANCE: invention can be used for prevention of neurological complications of the grave forms of adolescent idiopathic scoliosis surgical treatment with the use of segmental endocorrector and intraoperative Codivilla's extension. The essence concludes pre-surgical vertical traction with the load equal to the patient's weight. The anterioposterior radiographs of the scoliosis arc and an assessment of the neurological status are performed. If a neurological deficit occurs, the intraoperative traction is not performed, and test with the awakening is carried out directly after hardware. If a neurological deficit is absent, the skeletal traction is performed out of skulls and epimalleolar bones with a load equal to 20 kg after the ventral and notal mobilisation. The anterioposterior radiograph of the major scoliosis arc is performed. The obtained deformation angle is compared with the vertical traction angle and if the deformation angle decrease is less than 10°, the segmental instrumentation mounting is performed without test of awakening. As the angle of deflection decreases by more than 10° the test of awakening is performed and if the neurologic deficit is absent the segmental instrumentation installation is performed.

EFFECT: method prevents the development of complications when severe idiopathic scoliosis surgical treatment is performed.
