METHOD OF SURGICAL MANAGEMENT OF POSTTRAUMATIC TESTIS RUPTURE WITH FREE AUTOGRAFT OF PARIETAL LEAF OF VAGINAL TUNIC OF TESTIS Russian patent published in 2011 - IPC A61B17/00

Abstract RU 2417766 C2

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to surgical management of posttraumatic testis rupture. The method involves layer-by-layer opening of a cavity of a vaginal tunic of an injured testis. Blood, loose necrotising tissues are removed. The testis is mobilised with transaction of a Gunter ligament and dislocated to the wound. It is followed by saving resection of a nonviable testis parenchyma bulging in the wound. A flap is cut out of the parietal leaf of the vaginal tunic for creating a free autograft with its dimensions exceeding a defect of a testis albuginea by 5-10 mm. The free autograft is turned over and applied with its external anatomic surface inside to the testis parenchyma on a surface of the defect of the testis albuginea. The autograft is fixed freely along the perimeter of the rupture of the defect of the testis albuginea by separate long-absorbable interrupted suture over 3-4 mm with using an atraumatic needle pricked in at 2-3 mm from the rupture border. The procedure is completed by Winckelman's closure of the remained tunics of the testis, bed drainage. The incisional wound is closed layer-by-layer tightly, and a scrotum is placed in a suspensory.

EFFECT: method allows preserving the reproductive function.
