METHOD OF ENDOSCOPIC PLASTY IN CASE OF INGUINAL HERNIAS Russian patent published in 2011 - IPC A61B17/00

FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to herniology. Method includes installation of three trocars of required diameter in abdominal cavity and passing through them required instruments. Peritoneum is dissected in arch-like way above lateral and medial inguinal fossae with skirting of the latter. Peritoneum is separated from underlying tissues downward with isolation of hernial sac and formation of bed for mesh prosthesis. Angle-like cut of peritoneum is made with placement of angle apex in the place of projection of spermatic cord elements on the plane of internal inguinal ring and orientation of angle sides downwards below inguinal ligament in such a way as to limit part of peritoneum above iliac vessels, deferent duct and testicular artery and vein, which is not separated from said underlying structures. As prosthetic material used is mesh prosthesis without cutting out with anti-commissure layer, which is oriented in direction towards abdominal cavity and fixed with clips on perimetre with restoration of peritoneum above inguinal fossae. Separated from underlying tissues parts of peritoneum on the edge of made angle-like cut are additionally fixed with clips to mesh prosthesis. For inguinal hernioplasty polypropylene mesh prosthesis can be used, to which fixed is fragment of film material possessing anti-commissure properties of desired size in such a way as to exclude possibility of contact of said mesh prosthesis with organs of abdominal cavity in zone where iliac, testicular vessels and deferent duct are located.

EFFECT: method makes it possible to reduce trauma of deferent duct, testicular vessels, excludes their involvement into cicatricial process.
