SUBSTANCE: invention refers to medicine, namely to surgery, herniology, and can be used for surgical treatment of patients with oblique inguinal hernias. Dissection is carried out at the base of the hernial sac on the upper border of the mediolateral dissection, followed by suturing, ligation and dissection of the peritoneum of the hernial sac with preservation of the distal part of the hernial sac. U-shaped flap with freely lying medial and lateral edges is formed from the distal part of the hernial sac and the underlying structures are preserved in the central part in the projection of the vas deferens, iliac vessels and testicular vessels. Mesh implant is placed on the U-shaped flap in the allocated space in the projection of the vascular structures of the vas deferens. Mesh implant is positioned at an additional point to the upper-lateral portion of the U-shaped flap.

EFFECT: method enables to prevent damage to the vas deferens and testicular vessels at the dissection stage, involvement of the vas deferens in the postoperative inflammatory process, probability of recurrence, due to absence of distal hernial sac, interposition of U-shaped flap of hernial sac between spermatic cord and mesh implant, additional position suture.
