FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to abdominal surgery of ventral hernias. Hernia sac is excised, sheaths of rectal muscles of abdomen are opened on both sides on medial edge of posterior leaf. Rectal muscle is separated in blunt way. Aponeurosis leaf is cut upward and downward: upward - to the level of xiphoid process, downward - below navel to visualisation of transverse fascia, transverse fascia is additionally separated 3-5 cm downward. Posterior aponeurosis leaves together with peritoneum are mobilised from lower muscle surface to linea semillunaris (Spigeli) from both sides. After that, between posterior leaves of aponeurosis of shealth of rectal abdominal muscles edge-to-edge prosthesis of lyophilised dura mater is performed by uninterrupted blanket atraumatic polypropylene suture. On entire perimeter of mobilised retromuscular space with not smaller than 5 cm both in length and in width overlap on dura mater on which prosthesis is applied, laid is synthetic implant without strain and wrinkling. Implant is fixed to posterior leaf of aponeurosis on periphery by uninterrupted whipstitch. Edges of anterior leaves of sheaths of rectal abdominal muscles are adapted on medial line by uninterrupted atraumatic suture by polypropylene thread edge-to-edge.

EFFECT: increase of hernioplasty efficiency due to combined replacement of anatomical defect of anterior abdominal wall tissues.
