FIELD: medicine.

SUBSTANCE: invention relates to medicine, oncology, and can be used for surgical treatment of liver tumours of central localisation with invasion into arteries of porta hepatic and non-uniform affection of their membranes, complicated by obstructive jaundice. For this purpose from cancer infiltrate hepatic artery and its branches are isolated. Unremovable tumour tissues and cells and superficial metastases are left on vessel walls. Hemihepatectomy is performed within healthy tissues. Tumour tissues and cells remaining on vessels after their isolation from tumour infiltrate are subjected to cryodestruction with liquid nitrogen by application method with rate of freezing 100±10°C/min at critical temperature of destruction -160±5°C with exposure 7±1 minutes and further spontaneous thawing at rate 20±5°C/minute. Immediately after it cryo-spraying on micrometastases of isolated from infiltrate artery is performed with freezing to the depth up to 2 mm.

EFFECT: method makes it possible to achieve radical destruction of cancer tissue and cells, remaining on vessels after their isolation from tumour infiltrate, without rejection of vessel wall tissues and formation of defect, it also makes possible to perform radical operation in non-operable patients without carrying out radiotherapy, increase survival with satisfactory quality of their life with small financial expenditure.
