SUBSTANCE: invention refers to medicine, namely surgical otorhinolaryngology and oncology, and can find application in surgical management of extensive laryngeal malignancies, including with underlying radical radiation chemotherapy. Substance of the method consists in autograft pre-fabrication with the use of a high-permeable structure made of titanium nickelide alloy. Titanium nickelide alloy is implanted into an abdominal rectus muscle above the lower epigastric vessels. A tissue section is laid on a thin muscle bundle and coated by a similar muscle bundle, and from above - with a cast solid plate (foil) made of titanium nickelide. The length of the pre-fabrication stage makes 3 to 4 weeks. Thereafter, the autograft is taken from the abdominal rectus muscle with separating and preserving the lower epigastric vessels. Recipient vessels are prepared when removing the laryngeal cancer within healthy tissues. Then, vascular anastomoses with the autograft vessels are applied. Then, the autograft is sutured to the defect borders starting from a mucous layer.

EFFECT: use of the given invention enables recreating a full-thickness laryngeal structure, reducing a probability of complications with underlying trophic and inflammatory changes caused by radiation chemotherapy, extending the range of laryngeal resection with repair in extensive locally advanced tumours.
