METHOD OF TRANSPLACENTAL ANTENATAL CATHETERISATION OF UMBILICAL CORD VEIN IN FETUS Russian patent published in 2014 - IPC A61B17/42

SUBSTANCE: invention relates to medicine, in particular to obstetrics and perinatology. Preliminary scanning is carried out in pregnant women at term of gestation from 18 weeks to term of delivery to determine location of placenta relative to uterus walls, location of umbilical cord and its vessels. In case, if placenta is located on front surface of uterus, preliminary premedication is carried out in from of bolus dosing of spasmolytic drugs. Then, after 20 minutes transcutaneous introduction of spinal needle, provided with Luer connector, is performed into umbilical cord vein. Control over introduction of spinal needle into umbilical cord vein is realised combining B-mode of ultrasonic scanning and colour dopplerography. After that, polyurethane catheter with external diameter 0.6 mm, provided on its free end with Luer connector for fastening syringes and infusion systems, whose length must twice exceed needle length, is introduced into orifice of spinal needle. After that, needle is removed. In order to test if spinal needle was introduced into umbilical cord vein after removal of mandrin from needle and confirm exact location of catheter in umbilical cord vein blood sampling in volume 1 ml is carried out.

EFFECT: method makes it possible to provide constant access to systemic blood flow of fetus, carry out durative intrauterine drug therapy, with total duration over 60 minutes repeatedly during several days; as well as avoid a number of complications: premature interruption of pregnancy, infection and detachment of normally placed placenta.
