METHOD FOR VIDEO-ASSISTED CLOSURE OF PERFORATED PYLORODUODENAL ULCERS THROUGH SINGLE LAPAROSCOPIC APPROACH Russian patent published in 2014 - IPC A61B17/00

Abstract RU 2521354 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, specifically to abdominal surgery. A transrectal incision 2.5-3 cm long is performed on an anterior abdominal wall in a projection of a pyloroduodenal ulcer perforation in the right hypochondrium 2 cm in the distal direction from the costal arch and 4 cm to the right from the median line. An anterior leaflet of the rectus abdominis sheath is incised through the incision. The rectus muscle fibres are drawn apart longitudinally. A posterior leaflet of the rectus abdominis sheath and an abdominal peritoneum is dissected. A device for a single laparoscopic approach is placed into the incision. A panoramic abdominal inspection and a perforation visualisation follow. A lavage is performed with abdominal exudate drying. The device for the single laparoscopic approach is removed from a wound, and a carboxyperitoneum is eliminated. The wound edges are drawn apart with Farabeuf hooks. The perforation is closed with separate interrupted sutures through a transrectal incision. The approach wound is closed in layers.

EFFECT: method enables eliminating an intracorporeal suture in closing the perorated ulcer of the diameter more than 1 cm, reduces intraoperative injuries.
