SUBSTANCE: invention can be used to determine the tactics of surgical revascularization of the brain when creating extra-intracranial microvascular anastomosis (EICMA). After suturing one branch of the EICMA, brain reperfusion is assessed using intraoperative magnetic resonance imaging in ASL mode. If an increase in blood flow in the revascularization area and distant areas of the middle cerebral artery basin is determined, a decision is made to complete the operation. If local reperfusion is determined in the area of revascularization with large areas of perfusion deficiency remaining without dynamics in the frontopolar, parietal or temporal regions, a decision is made to impose an additional anastomosis with the acceptor artery supplying the area of perfusion deficiency, avoiding revascularization of the arterial basin with signs of local hyperperfusion. If the absence of signs of reperfusion or a large area of perfusion deficiency is determined, a decision is made to re-revise the anastomotic area and apply an additional anastomosis.

EFFECT: method improves the results of surgical restoration of cerebral circulation using bypass operations by determining the presence of indications or contraindications for additional brain revascularization with a second donor branch, determining the localization of zones of residual hypoperfusion and signs of local hyperperfusion.
