FIELD: medicine.
SUBSTANCE: invention relates to traumatology and orthopaedics and can be used for treating of polifocal unstable pelvic ring injuries with open fractures in the range of the front semi-ring and divergence of symphysis. On the iliac bones it is necessary to form a proximal support consisting of two semi-rings connected by an extension-type brace or braces. On the thigh, on the side of the fracture of the cotyloid cavity, it is necessary to form a distal support. Proximal and distal supports are connected by rods. It is necessary to perform repositioning of displaced fragments of the front half-rings. Through intact soft tissues it is necessary to introduce into pubic bones threaded rods connected with shims, which are attached to the specified extension brace with a possibility of movement. It is necessary to perform a three-plane reposition of residual displacement by traction and turning using rods installed in pubic bones. After repositioning of the cotyloid cavity and the front half of the pelvis it is necessary to stabilise the device by installing a bracer, connecting the distal support and a semi-ring of the proximal support on the side of the cotyloid cavity without damage. Wound is closed. It is necessary to turn the patient onto stomach and to perform from Kocher-Langenbek incision a revision and metal osteosynthesis of the fracture of the rear edge of the cotyloid cavity, associated with the petrocotyloid fracture.
EFFECT: method provides for a full-scale repositioning, reduction of a risk of infectious contaminations of soft and bone tissues.
