FIELD: medicine.
SUBSTANCE: invention relates to medicine. Surgical instrument comprises part of handle, rod containing starting bar and end effector. End effector comprises support, lower branch, cross-linking and dissecting unit responding to longitudinal closing motion, which is part of handle and rod. Lower branch is configured to receive detachable cartridge in open position. Cartridge includes housing, set of staples, arranged in housing, and platform arranged on top of set of staples. Platform makes holes, each hole, in fact is placed on top of each staple. Tool includes fastening element made with possibility of movement, which is single whole with body of cartridge or is single whole with lower branch of end effector. Fastening element may be attached to support and include part for reception of tissue.
EFFECT: when receiving tissue, fastening element is pushed and moves in proximal direction inwards for additional coverage of tissue.
