ENDOVENTRICULAR PATCH PLASTICS IN CASE OF LEFT VENTRICULAR ANEURYSM Russian patent published in 2017 - IPC A61B17/00

Abstract RU 2613675 C1

FIELD: medicine.

SUBSTANCE: aneurysm cavity autopsy and debridement are performed. An intraventricular balloon corresponding to the volume of the left ventricle cavity and size depending on the patient's body size is placed. A purse-string suture is applied along the scar edge, with subsequent tightening. The patch is sutured to the wall defect by a twisted suture. Aneurysmal sac walls are sutured above the patch after purse-string suture tightening along the scar edge. At that, two Z-shaped sutures are formed in the area between the interventricular septum, rear and side walls of the left ventricle. The first Z-shaped suture is formed by injection of a needle with a thread into the septum and left ventricle rear wall closure area with the pitch at a distance of 0.3-0.5 mm with an outer wrap of the purse-string suture thread. After the thread is passed similarly at a distance of 0.3-0.5 mm from the first injection and pitch with thread increment of 0.3-0.5 mm forming the second line of the Z-shaped suture. Similarly, the second Z-shaped suture is made on the diametrically opposite side wall of the left ventricle.

EFFECT: method allows to improve the treatment efficiency by reducing the severity of postinfarction left ventricular remodeling processes, improving its contractile function and myocardial perfusion, as well as provide the absence of life-threatening ventricular arrhythmias.
