FIELD: medicine; cardiovascular surgery.
SUBSTANCE: at the first stage, after clamping the common carotid artery (CCA), it is crossed proximal to the end zone of the atherosclerotic plaque (AP). Eversion endarterectomy is performed from the CCA to the carotid bulb. Then, at the carotid bulb, the AP is cut off with vascular scissors, after which an end-to-end anastomosis is performed between the proximal end of the CCA and the CCA site from which endarterectomy was performed. At the second stage, the internal carotid artery (ICA) is cut off with a portion of the carotid bulb with AP. Then an open endarterectomy is performed from the carotid bulb and an eversion endarterectomy from the ICA. After that, the ICA is implanted in its original place with the formation of an end-to-side anastomosis.
EFFECT: method allows to carry out endarterectomy from the CCA and ICA with the diameter of the carotid bulb exceeding the diameter of the CCA.
