FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to cardiovascular surgery. Method of carotid artery bifurcation reconstruction in presence of hemodynamically significant extended stenosis of common carotid artery and ostial stenosis of internal carotid artery, in which the inferior thyroid artery, common, external and internal carotid arteries are separated, then the internal carotid artery is clamped in the distal direction from the stenosis, the common carotid artery is clamped in the proximal direction from the atherosclerotic plaque, and the external carotid artery is proximal to the inferior thyroid artery, followed by a diagonal arteriotomy distal to the stenosis of the internal carotid artery from its lateral wall with a transition to the medial wall of the common carotid artery, thus completely cutting off the common carotid artery from the bifurcation, then the atherosclerotic plaque is fixed, the common carotid artery is inverted in the proximal direction and the atherosclerotic plaque is removed, and then returning the common carotid artery to its initial state and performing its implantation in the previous place with creation of an end-to-end anastomosis.
EFFECT: declared invention provides the possibility of carotid artery bifurcation reconstruction in the presence of hemodynamically significant extended CCA stenosis and ICA ostial stenosis while eliminating the risk of developing restenosis, stroke and hypothyroidism.
