FIELD: medicine; cardiovascular surgery.
SUBSTANCE: common carotid artery, external carotid artery and internal carotid artery are clamped, the internal carotid artery is cut off. Moreover, the external carotid artery and the internal carotid artery are clamped 4 cm above the ostia. After clamping the arteries, the external carotid artery is cut off 3 cm above the ostia, while its distal section is stitched. Its proximal section is revised and then the internal carotid artery is resected by cutting off at the ostia and 2 cm above the ostia. Next, the common carotid artery is sutured at the site of cutoff of the internal carotid artery at the ostia, after which an end-to-end anastomosis is performed between the proximal section of the external carotid artery and the section of the internal carotid artery remaining in the wound.
EFFECT: method makes it possible to prevent removal of an atherosclerotic plaque in case of hemodynamically significant ICA ostial stenosis, which, in turn, reduces the risk of neointimal hyperplasia and thus reduces the risk of restenosis of the ICA and the need for reoperation.
