METHOD FOR SURGICAL ACCESS TO CAROTID AND SUBCLAVIAN ARTERIES DURING SIMULTANEOUS INTERVENTIONS FOR SINGLE ATHEROSCLEROTIC LESION Russian patent published in 2018 - IPC A61B17/00 

FIELD: medicine.

SUBSTANCE: access is formed under general anesthesia, when performing simultaneous carotid endarterectomy and subclavian-carotid transposition or carotid-subclavian artery bypassing. A cut is made from the sternoclavicular joint to the mastoid to dissect the skin, subcutaneous tissue and m.platisma. In the upper third of the wound, the sternocleidomastoid muscle together with the internal jugular vein are withdrawn outwards by a retractor. The distal common carotid artery, external and internal carotid arteries are selected. In the lower third of the wound, legs of the sternocleidomastoid muscle are separated by a retractor. If necessary, the omohyoid muscle is dissected at the tendon level after preliminary tack-up suture application. The anterior scalene muscle with the phrenic nerve and the internal jugular vein is withdrawn outwards. The first segment of the subclavian artery, vertebral artery and internal thoracic artery are isolated and tacked. At the first stage, subclavian-carotid transposition is performed, at the second - carotid endarterectomy from ICA.

EFFECT: method provides simultaneous execution of operations in case of ipsilateral lesion with simultaneous correction of failure in the carotid and vertebrobasilar basins due to access to the subclavian and carotid arteries.
