METHOD OF CAROTID ENDARTERECTOMY USING A FLAP FORMED OF AN OCCIPITAL ARTERY Russian patent published in 2019 - IPC A61B17/00 

FIELD: medicine.

SUBSTANCE: invention relates to medicine, specifically to vascular surgery. After separation and revision of common carotid artery (CCA), internal carotid artery (ICA) and external carotid artery (ECA) along medial edge of sternum-subclavicular muscle, occipital artery is separated, ligatured and cut off between ligatures. Presence of haemodynamically significant stenoses of ICA, CCA is established, and reconstruction is started. After arterial clamping, ICA is dissected from carotid bulb in oblique direction at acute angle to CCA axis and dissected along inner surface in distal direction until end of atherosclerotic plaque. ECA is dissected in a distal direction with transition to occipital artery. Endarterectomy is performed from the ICA, CCA, ECA with subsequent formation of anastomosis with a suture seam between the ICA and the posterior wall of the occipital artery flap with transition to the ECA-CCA.

EFFECT: method enables completely removing an atherosclerotic plaque which extends along the ICA distally to the carotid bulb by 15–20 mm and more, which improves the surgical efficiency.
