FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely, to abdominal surgery. Midline laparotomy and sanitization of the abdominal cavity are performed. Source of the infection is eliminated and the instillation and drainage device is installed. Vacuum-assisted laparostome is applied. Fractional lavage of the abdominal cavity is performed with an antiseptic solution in combination with local vacuum therapy. In this case, the antiseptic solution and the inflammatory exudate are removed from the abdominal cavity by means of a vacuum system in the regime of creating a constant negative pressure from 100 mm Hg up to 120 mm Hg, in addition to the period of fractional feeding of the antiseptic solution into the abdominal cavity, after which the apparatus for creating a negative pressure is included 15–35 minutes after the administration of the antiseptic solution to the abdominal cavity is completed. In this case, the antiseptic solution is injected into the abdominal cavity in a volume of 200–600 ml in 4–8 hours. An instillation-drainage device consisting of an antiseptic solution injection system is used. System includes 6 elastic perforated silicone drainage pipes with a diameter of 5.5 mm to 10.3 mm, connected to a splitter made of silicone, which has an additional input with a diameter of 10 mm to 23 mm for connecting the central tube used to feed the antiseptic solution into the abdominal cavity and the vacuum aspiration system.
EFFECT: method and device allow to reduce the number of sanitizing operations, reduce the value of intra-abdominal pressure, the frequency of suppurative complications from the anterior abdominal wall, as well as the frequency of intercellular abscesses and intestinal fistula.
