FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to maxillofacial surgery, and is intended for use in the treatment of patients with partial tooth loss in the anterior section using dental implants. Removal of the tooth and pathological tissues, the treatment of the hole with an antibiotic, the formation of the bone bed in the interior of the alveolar process, the marking of the position of the implant and its installation, the filling of the free space between the walls of the alveolus and the implant with biomaterial stimulating osteogenesis. After processing the well with the antibiotic, 0.9% solution of sodium chloride. Immediate dental implantation is carried out simultaneously with soft tissue plasty. In this case, the preparation axis is displaced by 20-22° from the axis of the alveolus of the removed tooth. Install the implant and the plug, in the area of the marginal gingiva from the vestibular side. Carry out an intra-groove incision in such a way that a “pocket” is formed inside, bounded on one side by the periosteum, with the other mucosa. From the donor zone on the upper jaw - the hard palate or the upper jawbone area - the combined free gingival graft is sampled in such a way that it consists only half of the connective tissue and the other includes the epithelium. With the help of ceramic boron, remove the surface layer of the epithelium by 2 mm from the edge, obtaining an autotransplant with three zones: an epithelial strip in 2 mm, a diepithelial and connective tissue zone. Place the transplant in the formed “pocket” of the recipient bed and fix the nodular seam to the mucosa from the vestibular side so that the graft is immersed to the epithelial stripe. Space between the vestibular wall and the implant is filled with osteoplastic material based on hydroxyapatite. Plug is replaced with a gingiva former. On mucous membrane impose a U-shaped seam.
EFFECT: method, through the use of a one-step method of implantation, reducing the degree of resorption of the vestibular wall, increasing the thickness, allows to shorten the period of treatment of the patient, to preserve soft tissue architecture in the area of the implant, thereby obtaining a more predictable aesthetic result of treatment.
