FIELD: medicine.
SUBSTANCE: invention relates to medicine, preferably to hepatobiliary surgery and oncology, and can be used to determine overall survival in patients with portal cholangiocarcinoma (synonymous with Clatskin tumor) after surgical treatment. Following factors of overall survival are determined: stages of disease according to TNM, the presence of tumor cells at the margin of biliary duct and liver resection, the degree of differentiation of tumor cells, the tumor growth form, presence or absence of damage to regional and distant lymph nodes, vascular, lymphovascular and perineural infestations, the spread of the tumor to fatty tissue and liver parenchyma, invasion of the bile duct wall and out of it. Assign scores to the obtained data: I-II stage of disease corresponds to 0 points, stage III – 2 points, stage IV – 4 points; tumor cells in the margin of resection R0 – 0 points, R1 – 2 points, R2 – 4 points; high degree of differentiation of tumor cells – 0 points, moderate – 2 points, low – 4 points; absence of defeat of lymph nodes, vascular invasion, lymphovascular invasion – 0 points for each sign, the presence of these lesions – 2 points for each sign; absence of perineural infestation, invasion of fatty tissue, invasion of liver parenchyma, complete invasion of the bile duct wall – 0 points for each feature, the presence of these signs – 1 point for each feature. Prognostic criterion P of overall survival in patients with portal cholangiocarcinoma is calculated after surgical intervention using the formula
where is the sum of the scoring significance of each factor; Fmax is the maximum value of risk factors points equal to 23 points. When value of P is obtained from 0 to 55 % inclusive, it is concluded that the probability of survival is more than 2–3 years. At P from 55 to 75 % inclusive – the probability of survival 1–2 years, inclusive. At P more than 75 % – the probability of survival is less than 1 year.
EFFECT: method provides increase in accuracy of determining survival of patients with Klatskin tumor after surgery due to a statistical analysis of the complex of clinical and morphological parameters of the tumor.
