FIELD: medicine.
SUBSTANCE: invention relates to medicine, particularly to cardiovascular surgery. Perform installation of a biological seamless prosthesis of the aortic heart valve “Perceval S”. In this case, pre-dissect damaged valves with calcification or other pathological process to the fibrous ring. Determine the diameter of the fibrous ring by selecting the meter included in the heart valve prosthesis set “Perceval S”. Then, overlapping points of the three guide sutures are determined by applying to the fibrous ring a positioning device for a seamless aortic valve prosthesis (AK), made in the form of a ring, diameter of which corresponds to the diameter of the implanted valve containing three marks along the circumference, located at distances from each other equal to 120°, and serving as a guide for selecting the points of overlapping guide seams. After that, being guided by the marked zones on the device, we are convinced that after implantation of the rack of the framework of the seamless prosthesis, the AK does not overlap the coronary arteries, superimpose the guide sutures on the fibrous ring of the aortic valve at selected locations from the ventricular side towards the aorta. Next, the prosthesis is assembled on the delivery device, while the free ends of the filaments of the guide sutures from the ventricular side are passed through the corresponding fixed loops on the prosthetic valve “Perceval S”. After that, along the guide yarn prosthesis is positioned at the level of the fibrous ring. Then remove the lower part of the prosthesis from the delivery device, which is fixed by a double annular cuff on the fibrous ring, then the rest of the skeleton that performs the root of the aorta. Next, the prosthesis is adapted with a balloon at a pressure of 4 atm for 50 seconds, by monitoring the correct position of the valve in the root of the aorta, after which the guiding sutures are removed.
EFFECT: method allows to provide opportunity to correct aortic defect of any etiology and avoid complications inherent in sutured valves, as well as complications associated with incorrect positioning of the aortic valve seamless aortic root, such as paraplanar fistulae, overlapping of the coronary arteries, and, as a consequence, avoid explantation of the valve.
